I REFER to the letter, “Give pharmacists dispensing rights” (The Star, Jan 7).
The ramifications of pharmacists having sole dispensing rights are important. Ideally, prescribing and dispensing are best kept separate, but we do not live in an ideal world.
The situation in Malaysia is not like in Britain where pharmacists are well-regulated, and adequate laws exist to protect the public – they cannot sell potentially dangerous drugs without a doctor's prescription.

In Malaysia, patients can easily obtain many potentially dangerous drugs such as antibiotics and steroids without a doctor's prescription from many pharmacies.
This is partly due to poor regulation of the pharmaceutical profession and partly to the ignorance of the public. They need to realise that the pharmacist is not the first person to seek advice from for diagnosis and treatment of disease.

Here is a real-life example: In trying to save the cost of medical consultation, patient X goes to the pharmacy and asks for a cream for an itchy skin rash.
He does not mention – out of modesty – and is not asked where the rash is (it is in the groin). He is sold a potent steroid cream, which makes the rash much worse.
When seen in the clinic, he is found to have a severe groin rash due to ringworm that has been aggravated by wrong treatment.
The diagnosis was wrong, and hence the treatment made things worse.

If and when pharmacists have sole dispensing rights, many Malaysians will do what patient X did: seek medical help from a pharmacist first, because there is no consultation fee.
Pharmacists are not trained nor qualified to diagnose and treat disease.
Diagnosis requires a thorough patient interview and a thorough physical examination.
Correct treatment can only take place after accurate diagnosis.
When patients shop around, are sold strong medications from different pharmacies without a prescription and take them in wrong combinations unsupervised, there is a risk of dangerous – even fatal – drug interactions.

This is the response from the MPS (Malaysian Pharmaceutical Society)

It is always the same complaint by doctors against pharmacies. They will mention
1. Pharmacy are not regulated
2. Pharmacy sell medicines without doctor prescription
3. Pharmacist in Malaysia are not trained like those overseas4. There is lack of enforcement in regulating pharmacy.

All the above are untrue as general statements and the Society had refuted these many times in the papers and yet some one will come along writing the same thing as if he knows the whole situation and offer tunnel-vision advice based on out-dated and bias outlook.
On the doctor side they will say:
1. doctors are trained for diagnosis
2. doctors know about drugs

It is noticed that it is always the individual doctor that writes on this issue. The MMA is quiet. On the other hand the MPS had to come up and respond to the issues raised. Is there not any pharmacist out there who are passionate and concern enough about the profession to put forward a reply ?
There is one area which doctors can claim to be better than pharmacist and there is no argument about it - They complain better and they complain more.

18 October 2005 - A Mark in My Life

18th October 2005 made a mark in my life…I was trilled when my sister sms me that I’ve received a letter at home…I guess what I’ve been waiting for the pass few week is finally here…she then called me after a while and told me that my scholarship application has been ACCEPTED!! My mind was blank…I don’t know who to thank and who I should share this news with…frankly speaking…I am not happy or excited…but I m relieve…really relieved…finally I can help loosen my father’s burden…I quickly sms few of my close friends and share the news with them… Even though, the amount that I am receiving is not much…but I am really grateful…It will definitely help me…Thanks to God…thanks to all those that have supported me during my ‘down’ time in 2003… Kai Shing and Kai Ling…Mrs Cheng…Thanks to all my friends now…Min Choo and all those that cared about me….

Last night I couldn’t sleep…a lot of things comes into my mind…all those that I’ve gone through from my primary school till now…the ups and downs….since young I’ve been studying hard and dream about getting a scholarship to finance my education…that’s my dream…frankly speaking getting good results I not my dreams…what I want is something more practical that can help me and my family…you could say that I m money-minded but that’s how life goes…I’ve been through it…I’ve feel it…and I know…seeing people with poorer results and better financial condition succeed in getting scholarship is indeed gives me lots of pain…I’ve failed few times but I guess I just never gave up…Finally after going through so much…my dream came true…I guess yesterday I finally succeed in my life…for once…I succeed!!

Alan in a state of euphoria!!

We went to Kai Shing House on the 9th of October for house warming...we ate, drank and sing some songs....everything was pretty normal until Alan got in to a state of euphoria.

Euphoria is a feeling of great happiness or well-being, commonly exaggerated and not necessarily well founded. It is usually assosiated with use of drug abuse...hahaha...which in Alan case i think it is due to excessive alcohol abuse.

At first he was seen resting and kinda bored after taking a heavy lunch.

After a while, he started to sing...

He starts to put in more and more emotion.

After tired of singing and drinking...he got tired...and he sits quietly on a nice comfortable chair.

Suddenly, he starts to make some noice.....

He starts to laugh!!

and laugh!!..non-stop!

After about 15 minutes of non-stop laughing...he finally stops....you can be sure when you see the satisfaction on his face......

Been having tests for the past few week...tired and bored...fortunately its going to end soon...semester going to end and guess what...the study break is approching...that's even worst....but i guess..no matter what.....life goes on...right?

Yesterday I went to watch this movie, "Cinderella Man" in One Utama....this movie is based on a true story and it is certainly a movie that everyone should watch...It is a movie that one will clap right at the end of the movie......

In the middle of the Great Depression, when an America in the grips of a devastating economic downturn was nearly brought to its knees, there came along a most unlikely hero who had crowds cheering on their feet-as he proved just how hard a man would fight to win a second chance for his family and himself.

That common-man hero was James J. Braddock-a.k.a. the "Cinderella Man"-who was to become one of the most surprising and inspirational sports legends in history. By the early 1930s, the impoverished ex-prizefighter was seemingly as broken-down, beaten-up and out-of-luck as much of the rest of the American populace.

Like so many others, Braddock had hit rock bottom. His career appeared to be finished, he was unable to pay the bills, the only thing that really mattered to him-his family-was in danger, and he was even forced to go on Public Relief. Extemporaneous Dispensing - Suppository

In extemporaneous dispensing practical this time we had the chance to make suppositories. This time we are make paracetamol suppositoris...it is mainly used for fever in kids especially.

Suppositories are solid, bullet-shaped preparations designed for easy insertion into the anus. They are normally made of a solid vegetable oil that contains the medicine. The suppository dissolves at body temperature and gradually spreads over the lining of the lower bowel, where it is then absorbed into the bloodstream.

I never expect that this practical is so tedious...hahaha..we ended up finishing the practical about 7 pm...just to make 6 tiny little 'bullet'..we took about 4 and a half hours....

Cocoa butter....this is the main base in the suppository....it will melt when the suppository is inserted in to the anus.

Paracetamol..the active ingredient in the paracetamol suppository...it is analgesic and antipyretic.

Silica Gel..it is used to give texture to the suppository....so that it wun break when you touch it...

The balance for us to weigh the material and some aluminium foil to wrap the final product.

Finally i have some free time to update my blog...i think my blog had been left collectiong dust for some time already...

Lately i have been having lots of PBLs sessions. I had 2 consecutives PBLs last week...and this week even worsts...i had 3...i m glad that its over...however, i must admit that we can learn a lot through a PBL session...

PBL is Problem Based Learning...It is divided into 2 session...In PBL 1, we will be given a case presentation...usually the case will be based on a certain disease...we will have to discuss the learning issues that we want to venture in....then in PBL 2, each of us will be presenting the learning issues that have been assigned earlier...PBL involves a lot of reading...one must really understand whatever topic that he or she will be presenting...if not you will find yourself in the pool of blood....you will be shoot by tons of questions..from the lecturer or even your PLBmate.

I have been sick for the past few days...it has been a very long time since the last time i really fall sick...it started with fever...then bad sore throat...it was really bad till i have to see doctor...paid RM32 for the erythromycin (antibiotics)..all the others medication given by the doctor i could get it in pharmacy...surely it will be cheaper than that...but what to do....aiihh...lots of people around me fall sick too....so many people coughing during lecture...i guess i have to start taking care of myself well...

I guess i will be very busy starting from now...there are tests coming soon and lots of assignment to hand in...hehehe....i hope i can manage...i will try and i will do my best.....

Extemporaneous Dispensing Lab - Suspension

Extemporaneous Dispensing allows us to make some simple medication. This time we have the chance to make some suspension. Suspension is a dosage form in which the solid particles are suspended in the liquid phase. Most simple and common example of suspension is Callamine Lotion, which i will be making....

The callamine powders added bit by bit into the chlorofom water.

We made three products. Callamine lotion, Magnesium trisilicate mixture and Kaolin mixture.

Pharmaceutical Analysis Lab - Thin Layer Chromatography

We did this chromatography lab session on the 28 July 2005...

Chromatography is a method of separation in which the components to be separated are distributed between two phases, the stationary and the mobile phase.

First, we are given a Chromatogram sheet (20 x 20 cm).

These are the two types of developing chamber that we are using. Both containing developing solvent which is propanol : water (70 : 30 v/v). The solvent is made sure to be in a saturated condition by applying filter paper on the sides of the chamber (done by the lab assistant).

We are doing chromatography on two substance, amino acids and colour dyes. The colour dyes were spotted on the chromatogram sheet.

This is how the spots of different dyes look like.

Hairdryer is used to evaporate the solvent.

Finally its done! I would say its not a very good separation. =p

For the amino acids compounds, Ninhydrin sprays is used to make the compound visible. Amino acids is not visible in the chromatogram sheet.

It has been a tired week for me...its just wednesday..3 more days to go..but i m dead tired...orientation is so damn tiring...havent have the chance to post some pictures of orintation...have no time to take any picture...lots of work to do...i hope i can post some picture in a few days time...there are 80 juniors...i would say that they are quite good already cause most of them join the orientation...really hope that they will enjoy it cause we have put lots of hard work and sacrify our holiday to plan the orientation for them...its not easy to do a orientation what more with so few of us in our batch...everyone's workload just doubled or maybe tripled....

The schedule for the orientation week:

Monday : Cartoon Day
Tuesday : Angel and Cupid Day
Wednesday : Handsome girl and Pretty boy Day (Cross Dressing day)
Thursday : Joker Day
Friday : Begger Day

Tonight we will have the chance to see some guys dressing as a girl...there will be some Sexy Nurse, Belly Dancer, Japanese Schoolgirl and etc...i will try to take a few snap and post... =)

A True Story About 2 Sons

Once, there was a family known as Malaysia. In 1985,they were blessed with their first born, Proton. The family loved Proton all they can.They gave Proton everything. They protected Proton from the bigger boys in Kindergarden. They told Proton, "You're young, you need protection from them. It's okay to make mistakes, you're young. Even if you don't learn from mistakes...it's okay...you are young."

Proton became spoilt. He always depended on parents and used up all his allowance on useless things. When he had to do his chores, he paid people to do it and the work is lousy, causing the parents having to call repairmen to fix all the mess that he made.

Anyway, when Proton was 10 (but he still acted like he is 1 or 2 years old), he had a younger brother, Perodua. At first, everyone thought this younger brother will be like Proton. Anyway, this new baby was more handsome and looks smarter than Proton. Malaysia knew there is a possibilty Perodua could be smarter than Proton. Since they love Proton so much, they decided that Perodua can only learn a few things. Cannot learn the same thing as Proton. This is so that Perodua wouldn't beat Proton at what Proton is doing.

Although Perodua was also protected by Malaysia, he learn to be independent very fast. He made good friends with Daihatsu, who had an elder brother Toyota. The two of them helped Perodua in everything.

Perodua, being independent and smart, learnt a lot and in a short while, was becoming as good as, if not better, than Proton.

Malaysia realized that Perodua was becoming better than his brother. Although proud of Perodua, they still love Proton best. They paid Lotus(which is older and well known worldwide) to be friends with Proton.Using this advantage, Proton mentioned Lotus everytime he wanted to tell people about himself. On this own, he was still the same.

Moreover, Proton is always not updated. When doing his work, he still uses the typewriter and refuses to use the computer. Perodua, so much younger, used the computer, with broadband for his work. Perodua was improving so much that Toyota has assigned him with his work, codename Avanza.

2005. Proton is 20 years old, Perodua only 10. It was obvious that Perodua was the more successful of the two siblings. What is worse,Proton said he still need his parents to help him foranother 10years.

He still need to protection from the big boys. In May 2005, Perodua was about the show Malaysia his best work so far...codename Myvi. Proton, being jealous...quickly came up with an unprofessional remark about Perodua's work and show his work at the same time asPerodua on purpose. The future of Malaysia and her two kids are still unknown. But it would be obvious that Perodua will shine, leavingProton crying. It would be a wonder if in 2015, Proton will ask Malaysia for another 10 years of help and protection...maybe this time...protection from Perodua as well...........

Went into this toilet in Highlands Hotel in Genting. Its a bit special so i decided to take a few shot.

Its covered with plastic..each new user can turn the paddle on the right to get a new layer of plastic...cool huh?...anyone seen something like this before?...can save the time to wipe it with tissues or toilet paper...

Here is the instruction teaching people how to use it....

Family: Lucanidae

Origin: Malaysia

Notes: Stag beetles, family Lucanidae, are found worldwide but are most abundant in the tropics. These beetles can be told apart from other beetles in possessing large mandibles in the males and having elbowed antennae. Adults are found feeding on sap and rotting fruit. The larvae are grubs that develop in rotting logs. Adults of most species are capable of inflicting a painful bite if a finger is accidentally placed in their mouth but are considered harmless. Stag beetles are highly prized by collectors and in Japan are kept as pets.

Free movies on GSC One Utama Opening.

Went to One Utama on the 21 and 22 June 2005 for the GSC Opening. Its not because of the opening ceremony but its because Foot Seng has got lots of free movie tickets. Have watched lots of old movies that i've missed during my finals in Uni.

A visit to my old school.

My holiday has started for a while..have not have the chance to visit my old school, St John's Institution..so today i've spend a morning in the school...it has bring back lots of old memories that i had during my form 6...had a good talk with my teachers and met some of my juniors...too bad people from my batch seldom go back and visit their alma mater...

St John's Institution my alma mater.

It is exactly one hundred years ago when Bro. Julian Francis, Bro. Andrew Corsini and Bro. Alexander first found their way up what must have been then a rather wild and forbidding Bukit Nanas. On the 18th January 1904 St. John’s Institution was born when 40 pupils in three small classrooms completed the roll of SJI.

Great enterprises always had humble beginnings. Among the many splendid educational institutions in our fair land of Malaysia, there are not many which can look back over one hundred years of continuous, dedicated, service to the youth of our nation.

The La Salle Brothers and their teachers believed that the school should always have as its aim that the young man leaves it as a harmonious personality not as a specialist. It is essential that the student acquire an understanding of and a lively feeling for values. He must acquire a vivid sense of the beautiful and of the morally good.

This essentially is what the La Salle Brothers and their teachers have imbued on the thousands upon thousands of Johannians over the last 100 years. This is the type of holistic education that distinguishes St. John’s Institution from others. We have received the best of the best.

Considering the fruits culled in all these years of effort, the picture is very satisfying. Many scores of past pupils of St. John’s today give useful and enlightened service to the community in general following the training given by the Brothers and teachers.

There are Johannians in all walks of life. Many of its past pupils prominent in the learned professions, Government service and the commercial world. Just to name a few beginning with Royalty, the Sultan of Selangor; the Raja Muda of Perak Dr. Raja Nazrin ibni Sultan Azlan Shah; we are proud to have the number two in the country in YAB Dato Sri Najib bin Tun Abdul Razak, the Deputy Prime Minister of Malaysia; Datuk Hishamuddin bin Tun Hussein Onn the Minister of Youth and Sport.

Only two Malaysians had reached the summit of Mt. Everest and one of them is Johannian N. Mohanadas. In every field there are outstanding Johannians, be it in the professions, sports, theatre and the arts.

The Johannian spirit lives on and we pray it will continue to guide the destinies of St. John’s Institution and will inspire the future generations of Johannians as it has guided and inspired those of the past.

On this auspicious occasion of the one hundredth anniversary we have the opportunity and the happy duty of saluting and thanking all who have contributed towards making St. John’s Institution what it is, of venerating the memory of our inspired predecessors.

Once a johanian, forever a johanian. The johanian spirit will continues in me.

Had a bad night last night..i've always wanted peace in mind and i lost it last night...hate this feeling...i've lost this feeling for 4 years...never hope for it to come back...but i felt it again last night...but i guess life does not always work out the way we alwways wanted to...if not its not called 'life'..i guess i just have to work harder to overcome the feeling of unpeacefulness...i just have to work harder....i will and i have to....

ps: to all my friends out there who is struggling to survive in this world, take a deep breath and face the life...hopefuly it will be better....
